VO2 Max as the Strongest Single Predictor of Long-Term Healthspan

VO2 max quantifies the maximum volume of oxygen an individual can transport and utilize per kilogram of body weight per minute during maximal exercise ($mL/kg/min$). It reflects the integrated functional capacity of the pulmonary, cardiac, vascular, and muscular systems.

Longitudinal cohort research initiated by the Aerobics Center Longitudinal Study (Blair et al., JAMA, 1989; 13,344 participants followed for an average of 8.1 years) demonstrated that physical fitness is strongly and inversely related to all-cause mortality. Blair et al. observed that the steepest reduction in mortality occurred between the least fit quintile and moderately fit quintiles, establishing that even moderate gains in cardiorespiratory fitness confer substantial survival dividends.

2. Epidemiological Comparisons & Risk Ratios

In landmark cohort research from the Cleveland Clinic (Mandsager et al., JAMA Network Open, 2018; 122,007 consecutive patients undergoing exercise treadmill testing between 1991 and 2014 with a median follow-up of 8.4 years), cardiorespiratory fitness was inversely associated with all-cause mortality across all age groups with no observed upper limit of benefit. Elite aerobic fitness was associated with an 80% reduction in all-cause mortality hazard compared with low fitness (adjusted HR 0.20, 95% CI 0.16–0.24). Conversely, low cardiorespiratory fitness was associated with an adjusted hazard ratio of 5.04 (95% CI 4.10–6.20) compared with elite performers, demonstrating a risk profile comparable to or exceeding coronary artery disease, smoking, or diabetes.
